• We will contact you within five business days after receipt of all information.

Step 1.Understand the requirements

• This Form is for Instructors/Businesses who'd like to enter into a revenue-sharing partnership with the City of Kirkland's Parks and Community Services Department's Recreation Division.

Step 2.Download, print, and complete

• Download the Recreation Program Proposal pdf(PDF, 166KB), print, and complete.
• If you are unable to print the pdf, a blank form will be available at our two Community Centers and our Parks Office at City Hall.

• To receive a paper Form by mail, please give our office a call at 425-587-3330.

Step 3.Submit the completed Form

• You have one of three ways to submit your completed Form along with any supplemental information you'd like us to review.

2. Mail to Kirkland City Hall - Parks 123 5th Avenue  Kirkland, WA 98033
3. Hand-deliver to one of the three locations below during regular business hours (Monday - Friday, 8 AM - 5 PM).
   • Kirkland City Hall  (Parks and Community Services office in the lower lobby) - 123 5th Avenue,  Kirkland, WA 98033
   • North Kirkland Community Center - 12421 103rd Avenue NE,  Kirkland, WA 98034
   • Peter Kirk Community Center - 352 Kirkland Avenue,  Kirkland, WA 98033
